This course is frequently sought by massage therapists looking for massage doula training approved for continuing education credit. A Note on Credentialing Language

In 2022, the NCBTMB the national board that sets professional standards for the massage therapy profession retired its own use of the word "certification" for massage programs, and has since raised concerns about continuing education providers using that term broadly. A growing industry standard now holds that the word "certification" or "certified" should be reserved for credentials issued by independent organizations that administer formal examinations, require ongoing recertification, and maintain independent third-party oversight of their programs. This is the capacity of traditional institutional massage schools of which we are not. Nor are there many continuing education providers who are.

Because no national accrediting body currently exists that certifies prenatal massage therapists, massage doulas, or other maternal or women's health massage specialists, the Institute of Somatic Therapy is honoring this shift by moving toward updated credentialing language. We are now issuing Course Specific Specialist Certificates in place of the term "certification." Until the industry evolves further or establishes a recognized independent body for these specialties, we will be transitioning away from using that term.

Our credential represents your advanced training, clinical proficiency, and demonstrated expertise in working with these specialized populations. It is a meaningful mark of your commitment and we stand fully behind its value.

We recognize that most employers require a therapist to be "certified" to work with specific populations, such as pregnancy and labor support. We are sensitive to this and can help you navigate this requirement by providing a certificate that reflects the language your employer recognizes. As far as our marketing and course descriptions are concerned, we have had to shift away from using the terms "Certification in Labor Massage" or "Certified Labor Massage Doula Therapist" solely. This is something that practitioners across the country are navigating. Be patient with us as the industry standards evolve.

Pre-Requisite

Completion of the IST Prenatal Massage Certification series is required before earning the Massage Doula Specialist title through the Institute of Somatic Therapy. This three-part series covers Pregnancy Massage Myths, Prenatal Massage Fundamentals, and Prenatal Massage Techniques.

You may take this doula course as a standalone if you have earned prenatal massage training elsewhere; however, you will not be eligible to earn the IST Massage Doula Specialist title without completing the prenatal series through us. Why Become a Massage Doula?

A massage doula is a licensed massage therapist who combines the nurturing support of a traditional doula with advanced massage therapy techniques addressing both the physical and emotional dimensions of labor and delivery. This dual approach makes you an invaluable presence in the birthing room.

Course Overview

This course does not teach massage techniques those are covered in the prerequisite prenatal series. Instead, it builds directly on your existing prenatal massage training to teach you the specific knowledge, clinical thinking, and hands-on labor support skills that define skilled doula practice. You will study the stages and physiology of labor in depth, common interventions and complications, positioning strategies, and the full arc of what a massage doula does before, during, and after each contraction and between them.

The course also includes a detailed section on marketing your doula services and building your practice, along with the business essentials, documentation, and professional standards that define excellent doula work.

About the IST Massage Doula Specialist Certificate

The Massage Doula Specialist Certificate is an advanced credential awarded solely by the Institute of Somatic Therapy, under the authority of Dr. Deanine Picciano, AP, LMT, upon demonstrated proficiency in the subject matter and completion of the internship documentation process. This is not a generic course completion certificate. It represents a higher standard of engagement with the material and demonstrates your commitment to excellence in this specialty area.

Neither the NCBTMB nor any state massage therapy board makes a determination on the awarding of this specialty credential. The Institute of Somatic Therapy is an NCBTMB Approved Provider (#280672-00); this designation reflects the quality approval of our continuing education courses, not the awarding of this specialty certificate. No independent credentialing body currently exists for massage doulas as a specialty within the massage profession. This credential is issued by IST under our own established educational guidelines.

Earning Your Massage Doula Specialist Certificate Optional Advanced Track

Completing the online course and passing the exam awards you 21 continuing education credits and a certificate of completion, with no further requirements.

To earn the optional title of Massage Doula Specialist, you must complete the following:

1. Read The Birth Partner by Penny Simkin (student-supplied; new, used, or borrowed copies are all acceptable)
2. Attend three births as a doula intern, completing a birth record for each
3. Complete a structured self-evaluation for each birth reflecting on what went well, what was challenging, what questions your client had, and three specific things you will do better next time
4. Obtain a signed client evaluation and release for each birth, including contact information for verification purposes
5. Submit your documentation through the online application process along with a one-time $35 processing fee

IST provides the birth record form, which tracks labor progress, complications, and interventions. The birth documentation forms are located in the PDF section of your course materials log in, click "Access Course," and look to the right for the PDF files.

Please note: In-person attendance is required for internship births. Virtual or online births are not accepted as internship documentation. There is no expiration date or required renewal for this specialty title once earned. Neither the NCBTMB nor any state massage therapy board makes a determination on the awarding of the Massage Doula Specialist title.

Dr. Deanine Picciano, AP, LMT is the Director of Education at the Institute of Somatic Therapy, bringing over 30 years of experience in integrative and holistic healthcare. Her extensive background spans acupuncture and Traditional Chinese Medicine, massage therapy, craniosacral therapy, classical homeopathy, herbal medicine, yoga, functional medicine, and integrative women's health. Dr. Picciano maintains a private practice in Sarasota, Florida, specializing in women's and children's health, and has been teaching and mentoring practitioners for over two decades. She has developed and delivered continuing education courses nationally and internationally for massage therapists, acupuncturists, midwives, and physical therapists.

Institute of Somatic Therapy automatically reports completions to CE Broker in applicable states, provided your license number is on file in your account. These hours are reported under the "General" category if your state recognizes doula training as a valid category of continuing education. Scope of Practice Disclosure

It is the responsibility of each massage therapist to determine whether doula labor support services fall within your scope of practice in your jurisdiction. We are not currently aware of any states that offer doula licensure, nor of any states that exempt doula labor support from the massage therapy scope of practice meaning your massage license should cover this work in most jurisdictions. However, state laws are always subject to change. Your state massage board holds you responsible for knowing the laws that apply to you. Nothing on our website should be considered legal advice.

Frequently Asked Questions

What does "doula" mean, and what does a doula actually do?
"Doula" comes from the Greek word for "handmaiden." A doula remains with a birthing person through active labor until after the baby is born, the placenta is delivered, and the mother is resting comfortably. A doula provides three types of support: emotional encouragement for both the birthing person and their partner; physical comfort through massage, positioning, and counterpressure; and information to help the client understand the stages of childbirth, what to expect, and what options are available if complications or interventions arise.

What is the difference between a doula and a midwife?
A doula provides emotional, physical, and informational support to the birthing person she does not perform clinical examinations, check dilation, or deliver the baby. A midwife's role includes all of those clinical functions and the actual delivery. In essence, a midwife takes the place of an obstetrician. A doula is focused entirely on the comfort and support of the mother and her partner; a midwife is focused on the medical care of both mother and baby.

Why must I complete IST's prenatal series to earn the Massage Doula Specialist title, even if I trained elsewhere?
This program was originally designed as one integrated course. The prenatal series is the clinical foundation upon which the doula training builds. We have seen prenatal courses that are a fraction of the depth ours provide missing the physiological changes of pregnancy, their impact on every body system, and a thorough study of contraindications. We cannot be confident that a potentially incomplete prenatal foundation will produce the same level of clinical readiness in the labor room. For this reason, we offer the doula course as a standalone for CEs, but will not award our specialty title without ensuring full training to our standards. Even with prior prenatal training, completing our series adds significant value: 28 additional CE credits, at most the cost of three massages, and the ability to earn our doula specialist title. We do not make exceptions to this policy.

Does this course qualify me as a "true doula," or is a massage doula a different thing?
A Massage Doula Specialist is, in our view, a stronger credential than a standard doula title. Why? Because a massage doula is a licensed massage therapist who is also trained in prenatal massage, postpartum massage, and doula labor support and has the option of adding infant massage training. As a client, who would you prefer: someone who can massage you throughout your pregnancy, be present in labor as both a massage therapist and doula, provide postpartum massage, and potentially teach you to massage your infant? Or someone who can serve as a doula in labor alone? A massage doula brings far more to the table.

Will this credential be recognized by DONA or other doula certifying agencies?
No. We are not affiliated with DONA, ICEA, CAPPA, ALACE, or any other doula certifying agency. Those organizations are not specifically designed for massage therapists and do not award massage therapy continuing education credits. Our three-part series trains you in prenatal massage, massage and doula support through childbirth, and postpartum massage a scope that exceeds what any general doula certification program provides.

I have already attended several births. Can those count toward my internship?
No. The purpose of the internship births is specifically to practice the techniques taught in this course. If you have not yet studied this material, it is not reasonable to assume those techniques were applied. All three internship births must occur after completing the course.

Do I need a separate license to work as a doula?
We are not currently aware of any states that require a separate doula license, nor of any states that exempt labor support from massage therapy scope of practice. Your massage license should be sufficient in most jurisdictions. However, state laws change and your state board holds you responsible for knowing what applies to you.

How long do I have to complete the course?
You have one year from enrollment to complete the course and any internship documentation. If extenuating circumstances prevent completion within that period, contact us. You may reinstate the course for an additional year for a $35 reinstatement fee.

Can massage therapy students enroll before becoming licensed?
Yes. Many students begin continuing education coursework near the end of their training. You may enroll and study at any time, but you may not practice the techniques professionally until you hold whatever license your jurisdiction requires.

Do I have to be a licensed massage therapist to enroll?
Our courses are designed for licensed and certified massage therapists. Non-licensed individuals may enroll for personal enrichment but may not practice professionally without the required license. Doulas and other prenatal health providers may use techniques that fall within their own scope of practice and licensing authority, but may not represent themselves as massage therapists or massage doulas without holding a massage therapy license.
